Things you need:
- Last 4 Digits of Social Security Number
- Medical Insurance Info
- Copy of Birth Certificate
- Registration fee (cash or check only):
- $50 – 4-6 yr. olds
- $65 – 7-12 yr. olds
- $75 – 13-14 yr. olds
- Mandatory Fundraiser
